COVID-19 RESOURCES:

The link below provides information and tools to help patients, families, and individuals with the advance care planning process.

For Individuals & Families

Who would make your medical decisions if you were unable to speak for yourself? The Larimer Advance Care Planning Team can guide any person, 18 and older, in creating a plan for future medical care that represents your values and what is important to you - at no cost.

We can guide you in:

Choosing treatment options you would want or not want should you be diagnosed with a life-limiting illness

Sharing your values and wishes with your loved ones

Selecting a health-care agent and understanding life-sustaining treatments

Putting your plan in writing and sharing it with your family and medical providers

Time to update your advance care directives?

We offer complimentary services to aid in reviewing and updating your preferences, notarizing your documents and distributing your advance care plan to local health agencies so they are available when they are needed.

For Medical Providers

The Larimer Advance Care Planning Team can provide complimentary services to help you take your advance care planning practices to the next level  to better serve your patients.

We can help to streamline your operations and maximize staff resources by offering the time and the one-on-one support your patients need in completing their advance care directives.

Our team can:

Provide customized trainings and educational seminars to grow your team's knowledge in order to move patients toward completing advance care directives
