We offer complimentary services to aid in reviewing and updating your preferences, notarizing your documents and distributing your advance care plan to local health agencies so they are available when they are needed.

We offer regular appointment times at the Fort Collins Senior Center on the second Tuesday of every month from 9am-12pm!
