The Larimer Advance Care Planning Team (ACP Team) is a collaborative effort, funded by the NextFifty Initiative, with contributions from the Health District of Northern Larimer County and community partners to engage and support individuals in completing advance care directives at no cost.
Our vision is that advance care planning becomes a natural and expected part of care in our community. We want adults to feel comfortable discussing their end-of-life wishes, understand their medical care options and express their values with their loved ones and medical providers, ensuring that medical directives are available when they are needed.
When the Larimer Advance Care Planning Team was formed, only a third of Larimer County residents had completed their advance care directives. However, only 8% had discussed their preferences for end-of-life care with their health-care providers. Since the team was formed, we have worked to bridge gaps in services, provide education and guide community members in making their medical preferences known and documented.
Since 2016 we have:
Educated and engaged nearly 9,000 individuals in advance care planning
Helped more than 3,000 individuals to complete their advance care directives and ensure they are filed with medical providers
Provided training for more than 800 medical personnel, helping them to better guide their patients in making end-of-life medical decisions
Provided nearly 300 presentations and workshops on advance care planning throughout Larimer County
Trained more than 150 caregivers in advance care planning
The Larimer Advance Care Planning Team is also working with UCHealth and Sharing the Care to launch the Patient Passport, a communication tool to help individuals with chronic and serious illness as they "travel" through the health-care system. This tool is a compilation of key information that better communicates who a person is by documenting essential health information, care preferences, and advance care directives so this information is available in emergencies and across transitions of care.
The Health District of Northern Larimer County is proud to partner with NextFifty Initiative, UCHealth's Aspen Club, Palliative Care Team at UCHealth, Associates in Family Medicine, Columbine Health Systems, Larimer County Office on Aging and Sharing the Care Campaign to enhance advance care planning in our community.
The Health District is a public agency that provides residents of northern Larimer County with dental, mental health, preventive health and advance care planning services, in addition to connecting people to more affordable prescription and health insurance options.

2. Talk about your wishes - with loved ones, friends and your doctor
3. Fill out the forms - Medical Durable Power of Attorney and Living Will
4. Select and prepare the person who is named as your health-care agent
5. Share your plan - add them to your medical record; let your health-care agent, family and close friends know how to access the forms when they are needed
