Patient Passport Resources:

The Patient Passport is a free, convenient, portable, user-friendly tool (similar to a travel passport) used by people as they ‘travel’ through the healthcare system.  It helps communicate essential health information, special needs/preferences, communication approaches, and informed healthcare wishes to healthcare providers.

Its purpose is to help communicate essential health information to first responders and health-care providers so patient wishes and preferences are honored. 

The Patient Passport is primarily meant to be used by older adults (50+) and/or those with chronic and serious illnesses who may transition across different providers and healthcare settings.
