On- Site Signing Opportunities

While only the Colorado Advance Directive for Surgical/Medical Treatment (Living Will) requires two disinterested witnesses, because the formal requirements for completing advance directives differ from state to state, we highly recommend that all legal forms are properly witnessed and notarized. This increases the likelihood that they will be honored in other states you may travel to. The Larimer ACP Program will provide witnesses and Notary services during the signing days listed below or you can email and make an appointment to complete your documents at our office at 120 Bristlecone Drive.

On-Site Signing Opportunities:

City of Fort Collins Signing Days will be Wednesday September 28th from 1-5pm at 222 Laporte in the Colorado River Room and Tuesday October 11th from 11-5pm at the Health Fair at the Lincoln Center.
